If you are a business, or if you have your own Business Insurance, please
provide a Certificate of Insurance noting the following:

Commercial General Liability insurance subject to limits of not less than Five Million ($5,000,000) 
inclusive per occurrence. Coverage shall include but not limited to bodily injury, personal injury, 
property damage including loss of use thereof, contractual liability, products and completed 
operations liability and contain a cross liability, severability of insured clause. The Corporation 
of the Township of Georgian Bay shall be added as an additional insured but only with respect to 
liability arising out of the operations of the Exhibitor/Vendor. The certificate of insurance must 
also include the Event Name, Date and Address, as well as a 30 day notice of cancellation.

If you are not a business or do not carry Business Insurance, and need to obtain insurance for your 
participation in this event:

Please note:

If you are selling any of the following products, you will need to provide your own certificate of 
insurance as PAL excludes Products and Completed operations for the sale of any Skin Care products/Cosmetics, which is a requirement for participating in this event.
